Waiver and Release of Liability

Video/Photo/Audio Consent

I hereby knowingly release, and agree to indemnify and hold harmless, Women Wine & Waders, LLC, a West Virginia limited liability company, and its officers and employees from all liability, actions, claims and demands of any kind arising out of my participation in the activities including, but not limited to, claims for personal injuries, death and damages of any sort.

I fully understand and acknowledge the: (a) risks and dangers exist in my participation in fishing activities: (b) my participation in such activities and/or use of related equipment may result in injury or illness including, but not limited to bodily injury, disease, strains, fractures, paralysis, and other injuries that could cause disability or death; (c) these risks and dangers may be caused by the negligence of others, including the company, accidents, breaches of contract, the forces of nature or other causes. These risks and dangers may arise from foreseeable and unforeseen causes; and (d) by my participation in these activities, I hereby assume all risks and dangers and all responsibility for any losses and/or damages, whether caused in whole or in part by the negligence or conduct of the company.

I, the undersigned do hereby consent to the use by Women Wine & Waders, LLC of my image/voice as described below in videos, photographs, or audio recording reproduced either in whole or in part from the video, photograph or audio recording; these materials can be used for advertising, publicity, or any other purpose on behalf of Women Wine & Waders.

In addition, I waive all claims to compensation or damages based on the use of my image/voice. I also waive any right to approve the finished photograph or video or audio recording.

I understand that this consent is perpetual, that I may not revoke it, and it is binding.

I warrant that I am at least 18 years of age and that I have read this consent form and fully understand its contents.
